Minnesota Secure Choice

Accurate time reporting is an employment requirement and an important Medicaid program-integrity responsibility.

Leave, ESST & Accommodations

Employees should follow company notification procedures in addition to any applicable government application process.

Safety & Workplace Rights

Workplace Safety

Especially important for employees providing PCA, CFSS, 245D, IHS, Homemaking, Night Supervision, Respite, ICLS, or other Medicaid-funded services.

Electronic Visit Verification

Employees providing EVV-covered services must use the authorized clock-in/clock-out process and report technical problems or corrections according to company procedure.

Documentation

Service Documentation

Documentation must be accurate, timely, complete, and consistent with services actually provided. Never fabricate, backdate, or intentionally alter documentation.

Complete all orientation, person-specific, safety, mandated-reporting, infection-control, positive-support, EVV, and other training required for your position.

Confidentiality & Data Privacy

Protect client information. Do not share protected information, photographs, documents, or service details with unauthorized persons or on social media.

Program Integrity

Fraud, Waste & Abuse

Employees must promptly report suspected billing fraud, falsified timecards, services billed but not provided, forged signatures, false documentation, improper payments, or other suspected Medicaid fraud, waste, or abuse.

Never change service times, documentation, or EVV records simply to avoid an overlap, authorization limit, or billing edit.

Mandatory Reporting

Vulnerable Adults

Employees who are mandated reporters must follow Minnesota law and company policy when they know or suspect abuse, neglect, or financial exploitation of a vulnerable adult.
